Cohorts 1, 2, & 3 Implementation Plans

Implementation plan – Cohort 3

Districts in the first year of the grant are required to submit an online implementation plan outlining numbers of teachers participating in the program and professional development course work and when they will be completing courses. This form must be submitted by December 31, 2008. Please feel free to share this plan with any one in your building. We will be utilizing the information to provide the best support to aid in your implementation and meeting the requirement for your district. This plan is for the professional development online courses only. You are not able to save work in the form so it may be best to create and save your answers in another location prior to submitting.

Districts who have previously submitted an online implementation plan are asked to submit a brief update to their plan online by October 1, 2008.
